Advanced Care Plan Workshop

Venue: Paraparaumu Library, 9 Iver Trask place, Paraparaumu

At this workshop people will be provided with a ACP booklet and each page will be explained. Questions will be welcomed. There will also be an opportunity for people to write in the booklet or write a draft copy on the paper provided. The Age Concern team will be available to help individual people clarify what they want to put in their booklet. They are also willing to follow up with people who attend the workshop.
