Susan Bowden and Chris Milne will talk about needs assessment and care management. Susan is the Service Manager of Care Coordination in the Wellington Region.
The goal is to enable aging in place by avoiding hospital admissions, shortening hospital stays and delaying entry into residential care. Their team provide strategies to keep people well, maintain independence and engage informal support, while utilizing community based models of care.
