Planning ahead for the end of life is a powerful gift we can give to ourselves and those who love us. In the event that you become unable to speak for yourself, having a completed Advance Care Plan (ACP) makes it easier for everyone to know your wishes and honour them.

 

Your ACP is a record of what is important to you, what concerns you, what your care preferences are and how you want your last days to be. You can also record what sort of funeral you would like, where your important papers are, who you want involved in discussions about your health as well as stating any Advance Directives (AD).

 

During this practical workshop professional End-of-Life Doula Sherie will support you to; clarify your values for end-of-life care, become informed of different choices you can make and document your wishes for care and treatment at and beyond the end-of-life.

 

Workshop Objectives:

  • Learn about the options for end-of-life care, funerals and body disposition – including specific legal requirements in Aotearoa, New Zealand.
  • Evaluate your preparedness for the end-of-life and identify any gaps that need addressing.
  • Complete an Advance Care Plan and prepare related questions for your GP, family and others who may be involved in your care.
  • Gain confidence to begin open, meaningful conversations with family, whānau, or trusted others about your end-of-life wishes.
  • By the end of the workshop, participants will walk away with more clarity, peace of mind and a completed Advance Care Plan that reflects their values, wishes and priorities.
